Signalment: Name, Age, Sex, Castration Status, Species/Breed
Presenting Complaint: What the patient is at the clinic for.

Please make sure the client's address is correct (down to the P.O. box) and that the consent is properly signed off. Without the client's signature, we are not allowed to see or treat the pet!
Update March 2023: Make sure client makes a decision with their initials on how to proceed with spay if the intact female is discovered to be pregnant.
